CICV can't intubate, can't ventilate, woo! Your are in DEEP trouble!

這是每一個麻醉科醫師的夢饜，當插不進管，而且還無法面罩換氣時，他將面臨外科氣道的挑戰，而每年因為CICV而需要作外科氣道的比率在0.01~2/10000，也就是說，可能數年間，一個麻醉科師可能也『分』不到一個，所以對於這樣的窘境，與緊要關頭，多數醫師難免遲疑，猶豫，而這也就是災難的開始。個人在舊文困難氣道『真正困難在哪裡』？曾經有過論述，可以連結參考。英國的Royal Perth Hospital麻醉科，製作了一CICV 流程圖的訓練program，除了classical needle cricothyrotomy外，還包括各種狀況的氣道access，值得我們學習。
